PurposeTo encourage patients to access exercise related support to help reduce risks associated with their clinical condition.

How does it work? Patient groups are identified via the risk identification and stratification tool. Contact is made via letter and telephone call, and patients can go online to complete a questionnaire and referral to Active NoW – a new Norfolk and Waveney wide hub that can provide a range of exercise support options, from low level physical activity (walking clubs, etc) through to fully structured exercise programmes.

What is the cohort? Initially, the project focuses on 3 Practices within the Norwich locality, identifying patients at high risk of developing diabetes. It is anticipated that this will be extended to additional cohorts, e.g. patients with hypertensive.

Outcomes:

The Protect NoW scheme aims to enable a manageable increase in referrals to the newly formed Active NoW service so that patients can easily access a wide range of exercise support.

The NHS advise that exercise can reduce the risk of major illnesses, such as coronary heart diseasestroketype 2 diabetes and lower risk of early death by up to 30%.
