The NHS is adapting to meet the needs of its patients, and the way that patients can access primary care services has also been evolving.
The aim of this campaign is to make it easier and quicker for you to understand what services are available to help you based on your health needs – and help you to understand how you can get the best from these services.
Supporting you to make the best use of local primary care services will help you get the right help in the right place for your health needs. This will help improve your patient experience, and it also supports our local primary care providers.
