Proactive Falls Prevention

This project will involve Norfolk County Council contacting people who are currently receiving social care services, or those who have done so in the past, that have an increased risk of falling in the next 9 months. This will be based on a data modelling tool that applies an algorithm to create a prioritised list.   The intention is to offer these patients some proactive interventions to reduce the fall rate and to keep them independent within their own homes for longer.

The project will involve an initial call and referrals onto to other services, with follow up calls to check that any interventions are going well and to assess their impact on the person’s wellbeing.
