Shared Care Health Records (SCHR)
The SCHR is a digital tool which will align patient records from different Health and Social care organisations, enabling patient information to be visible to front line professionals in any organisation, who are involved in a patients care.
The LMNS digital team and the three acutes digital midwives will work collaboratively with the SCHR project team to enable other registered providers to have read access to a maternity service users record in real time.

Norfolk and Waveney Electronic Patient Record (EPR) system
Work has been underway within Norfolk and Waveney on procuring an EPR system on behalf of the three acute trusts.
The procurement process has now concluded and as a result an EPR system has been selected, which maternity and neonatal services are in scope.
This means once available the EPR will support and enable clinicians responsible in a service users care to access their record both inter departmentally and across other hospitals, including those across the boundaries.
The LMNS Digital team and the digital midwives will work collaboratively with NHSE and project teams within each trust to build and embed the maternity and neonatal modules into the EPR system.

Patient Portal
The Long-Term Plan states that ‘every person should have access to their digital record by 2023/2024’.
The LMNS digital team will work collaboratively with the three acute trusts digital midwives and other key stakeholders to meet this national target, by providing every child bearing person access to a digital maternity portal, which will contain all necessary content including patient record and information documentation in one easily accessible digital place.
The portal will not only benefit the service user but also the service provider.

It is anticipated this project will project will start on the system wide Electronic Patient Record (EPR) project has concluded.
Digitalised Personalised Care and Support Plans (PCSP)
The Long-Term Plan states that ‘every person should have access to their digital record by 2023/2024’.
Work is underway with maternity representatives from the three acute hospitals and other key stakeholders to develop a paper based PCSP.
Once the paper-based documentation has been embedded into trusts and the system wide EPR project has concluded work will progress on digitalising the PCSP documentation.
It is expected that the digital PCSP project will coincide with the Patient portal project with the aim of enabling all pregnant persons to access their digital maternity records and information as well as enabling trusts to provide care that is more personalised, and person focused.
