Purpose: The ICB’s Population Health Management (PHM) team is supporting Great Yarmouth Borough Council’s Community Voices project to increase uptake of the NHS Lung Cancer Screening (LCS) programme among marginalised and disengaged communities in Great Yarmouth. The project also aims to maximise opportunities for smoking cessation through the Swap to Stop scheme.
Objective: To support earlier diagnosis of lung cancer, when treatment is simpler and more effective, and to better understand why some eligible individuals have not previously engaged. The project will provide a softer, more personalised offer to help individuals understand the screening process and support them to book and attend appointments. Individuals will also be supported to link with appropriate services, including smoking cessation and wider lifestyle and welfare support.
How it works: The ICB’s Population Health Management (PHM) team carries out risk stratification using information from GP records to identify individuals who are eligible for the programme. The Community Voices team from Great Yarmouth Borough Council will use this information to contact eligible individuals and offer personalised support to attend lung cancer screening, as well as support to access smoking cessation and wider lifestyle and welfare services. This project does not make referrals; it focuses on engagement and support to attend screening.
