Purpose: To help people in Norfolk and Waveney understand Familial Hypercholesterolaemia (FH) and take steps to manage it. FH is an inherited condition that causes very high cholesterol levels. Around 4,400 people in our area may have FH without knowing it, which can greatly increase the risk of heart disease or stroke if left untreated.
Objective: To support individuals in finding out whether they may have FH and ensure they receive the right care. Early diagnosis and treatment can lower cholesterol, reduce the risk of heart attacks and strokes, and help people live healthier, longer lives.
How it works:
- People who may be at risk will receive a letter inviting them to visit a dedicated webpage.
- The webpage provides clear, easy-to-understand information about FH and allows patients to book an appointment with a specialist nurse.
- Patients will also receive a text reminder. If they do not respond, our Virtual Support Team will follow up with a phone call to assist.
- Once under the care of a nurse, patients may have genetic testing, and if required GP will receive advice on appropriate treatment for the patient, Patient will also receive guidance from the specialist nurse on talking to family members who could also be affected.
