Across Norfolk and Waveney our population is growing, people are generally living longer and the type of care they need is changing.
Advances in health care mean that people are generally living longer, some with complex and multiple long-term conditions that we could not treat in the past. Our system does not always deliver the joined-up care that people need to help them with the daily challenges they face living with multiple long-term conditions.
We know there are ways to improve how we care for people, and that our current services are not sustainable if they continue as they are now. We are working to make patient care better in different ways. We need services that better empower people to take increasing responsibility for their health and care and provide more care in communities.
We are focusing on key conditions including stroke, diabetes and mental health in order to deliver real, measurable change. Across these pages you will be able to see the work we have undertaken – and continue to focus on – to improve outcomes for local people and communities.
