Primary Care Networks

GP practices in Norfolk and Waveney are working together with community, mental health, social care, pharmacy, hospital and voluntary services in groups known as primary care networks (PCNs).

PCNs bring health care partners together to enable more personalised and coordinated health and social care services for people closer to home.

The aim is to improve care and health for people locally so they can stay safe and well closer to home, and also to make primary care services more resilient.

PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice.

Patients will continue to remain registered with their own GP practice, but working as part of a PCN allows GP practices in Norfolk and Waveney to share expertise and resources. You can see how the practices are grouped together on the following pages.
