Type 2 Diabetes in the Young – T2DaY

Purpose: To improve engagement of patients with Early Onset Type 2 Diabetes (EOT2D) with Type 2 Diabetes in the Young (T2DaY) reviews provided by their GP Practice. 

Objective: Contact all patients living with EOT2D to invite them to access Protect NoW information zone webpage including engaging videos led by NW ICB Speciality Advisor. The webpage will seek to set out the importance of proactive diabetes management in this cohort, providing details of what they can expect and how to arrange a review with their GP practice.    

Method: This Protect NoW project will invite patients (via SMS or letters as appropriate) to access a Protect NoW webpage which hosts educational information and videos from the Speciality Advisor, that aims to educate/inform on the importance of attending diabetes reviews, and what differentiates that of a T2DaY reviews to support in improving management of their Early Onset Diabetes.  

Introduction

Early onset type 2 diabetes affects people between the ages of 18 and 39. It can be more challenging to manage than type 2 diabetes that starts later in life. It may progress faster and lead to complications sooner. However, with the right support and care, you can effectively manage your condition.  

We have put together some videos to give you more in-depth information. These videos will take you through the specifics of managing early onset type 2 diabetes, the importance of attending type 2 diabetes in the young (T2DaY) reviews, and the support available to you.
